Medicare Advantage vs. Medigap: A Side-by-Side Comparison

Medicare Advantage vs. Medigap: How the Two Paths Differ
Key takeaways
- There are two main paths: Original Medicare (often with Medigap + Part D), or a Medicare Advantage plan.[1][2]
- Original Medicare has no yearly out-of-pocket limit by itself.[1]
- Medicare Advantage plans have a yearly limit, but it varies by plan.[1]
- Your best time to buy Medigap is your one-time 6-month Medigap window. Later, insurers may say no or charge more.[3]
- 55% of eligible people were in Medicare Advantage in 2026.[4] Popular is not the same as right for you.
- We don't recommend plans or companies. A SHIP counselor can help, free.[5]
The comparison table (original)
| Question | Original Medicare + Medigap (+ Part D) | Medicare Advantage (Part C) |
|---|---|---|
| Who runs it? | Federal Medicare; Medigap sold by private insurers | Private plans approved by Medicare |
| Doctors you can see | Any provider that takes Medicare | Usually a plan network |
| Yearly out-of-pocket limit | None in Original Medicare alone; Medigap pays some or most gaps[1] | Yes; amount varies by plan[1] |
| Monthly premiums | Part B + Medigap + Part D premiums[1] | Part B + plan premium (varies)[1] |
| Drug coverage | Separate Part D plan | Often built in |
| Prior authorization | Generally limited | Nearly all enrollees (99%) are in plans that require it for some services[6] |
| Dental, vision, hearing | Not usually covered by Original Medicare | Often offered as extras[6] |
| Travel / part-year in another state | Works with any Medicare provider nationwide | Check if the plan covers you there[2] |
| Can I switch later? | Dropping Medigap may be hard to undo[3] | Yes, each Open Enrollment (Oct 15–Dec 7)[2] |
You must keep paying your Part B premium on either path.[1] The 2026 standard Part B premium is $202.90 a month (higher with higher income).[1]
The Medigap timing trap
Your Medigap Open Enrollment Period lasts 6 months. It starts the first month you are 65 or older and have Part B.[3]
During it, an insurer can't refuse you or charge more for health problems.[3]
After it, insurers may deny you or charge more, unless you have a "guaranteed issue right."[3] Some states give more rights. Ask your State Insurance Department.[3]
This window happens once. It does not repeat every year.[3]
Which questions fit you? (original worksheet)
Lean toward thinking about Original Medicare + Medigap if you:
- Want any doctor that takes Medicare, anywhere in the U.S.
- Travel or live in two states.
- Prefer predictable costs and can afford higher premiums.
Lean toward thinking about Medicare Advantage if you:
- Are fine with a network and your doctors are in it.
- Want low premiums and extras like dental or vision.
- Accept prior approval for some care.

Why policy experts watch this
MedPAC and KFF report Medicare pays private plans about 14% more per person than similar people in traditional Medicare in 2026.[4] This affects taxpayers. It does not tell you which path is best for you.
